Whichever type of account you wish to open, you can follow this three-step process: test
- Download and read the Sound Mind Investing Funds Prospectus.
- Download and print the appropriate account registration form for the type of account you wish to open. For retirement and Education Savings accounts, there are also additional documents to download and read as described below.
- Complete the registration form, and mail it along with a check for your initial investment to:

Standard Account
Individuals, businesses, trusts, and non-profit institutions may open standard investment accounts. A standard Sound Mind Investing Funds investment account requires an initial minimum investment of $2,500. However, there is NO minimum investment if you sign up for an Automatic Investment Plan and arrange for a minimum of $100 to be transferred on a monthly basis to your Sound Mind Investing Funds account.

Traditional IRA
Please read the IRA Disclosure Statement (attached to the application) before opening an IRA. For tax year 2013, you may contribute up to $5,500 into a Sound Mind Investing Funds Traditional IRA ($6,500 if you are over 50 years of age). Individuals not covered by a retirement plan at work, and people covered by a retirement plan who meet certain IRS income guidelines may be able to deduct their contributions from their taxable income each year. When you retire, distributions from your IRA are generally taxed as ordinary income. More information is included in the IRA Disclosure Statement. Please call 1-877-SMI-FUND or consult a tax professional if you have any questions regarding IRA accounts.

* If you are not transferring an existing IRA, then simply complete the Traditional IRA Account Application.
* If you are transferring an existing IRA, then complete the following 2 forms:
1. Traditional IRA Account Application,
2. IRA Transfer Request Form.
(Please note that a recent copy of a statement from the existing IRA needs to be included with the 2 completed forms.)

Roth IRA
Please read the IRA Disclosure Statement (attached to the application) before opening an IRA. For tax year 2013, you may contribute up to $5,500 into a Sound Mind Investing Funds' Roth IRA ($6,500 if you are over 50 years of age). Roth IRAs differ from traditional IRAs. With a Roth IRA you cannot deduct your current-year contributions from your taxable income; however, upon retirement, distributions from your Roth IRA are generally not taxed. More information is included in the IRA Disclosure Statement. Please call 1-877-SMI-FUND or consult a tax professional if you have any questions regarding IRA accounts.

* If you are not transferring an existing ROTH IRA, then simply complete the ROTH IRA Account Application, following its instructions.
* If you are transferring an existing ROTH IRA, then complete the following 2 forms:
1. ROTH IRA Account Application,
2. IRA Transfer Request Form.

SEP-IRA Account
A Simplified Employee Pension (SEP) plan allows self-employed people and small business owners to establish SEP-IRAs for the business owner and any eligible employees. SEP-IRAs have specific eligibility rules and contribution limits (as described on IRS Form 5305-SEP); otherwise SEP-IRAs follow the same rules as Traditional IRAs. Instructions for opening a SEP-IRA Account:
* If you are establishing a new SEP-IRA plan, please complete the following 2 forms:
1. SEP-IRA Account Application,
2. IRS Form 5305-SEP.
* If your employer already has a SEP-IRA plan established and you want to open a SMI Funds SEP-IRA account to receive your contributions, please do

1. Read the IRA Disclosure Statement (attached to the application)
2. Complete the SEP-IRA Account Application (Form 5305 is not required).

Health Savings Account
A Health Savings Account is a savings/investment account that is paired with a high deductible health insurance policy to meet medical expenses. Money can be taken out of a health savings account tax-free providing it is used to pay for qualified medical expenses. Please read the Health Savings Account Disclosure Statement (attached to the HSA Application) for more details. Instructions for opening an Health Savings Account (HSA):

* If you are transferring an existing HSA, then complete the following 2 forms:

2. HSA Direct Rollover-Transfer Request Form.

SIMPLE-IRA Account
A SIMPLE-IRA is a tax-deferred retirement plan provided by some small businesses, in which both the employee and the employer make contributions. Please read the SIMPLE-IRA Disclosure Statement (attached to the application) for more details. 401(k), 403(b) or 457(b) Rollover
If you have a 401(k), 403(b) or 457(b)account with a past employer that you would like to rollover into a Sound Mind Investing Funds IRA, please review the Sound Mind Investing Funds IRA Disclosure Statement, complete the appropriate IRA Account Application form AND complete the IRA Rollover form. UGMA / UTMA Account
Uniform Gift to Minor (UGMA) and Uniform Transfer to Minor (UTMA) accounts provide you with tax advantages as you put aside savings for your children or other minors. To open an UGMA or UTMA account, simply use the regular Sound Mind Investing Funds Account Application and fill in the appropriate information under the "Gifts To Minors" section. Call 1-877-SMI-FUND or consult a tax professional if you have questions regarding UGMA / UTMA accounts.

Coverdell Education Savings Account
Education Savings Accounts are designed to be a tax advantaged vehicle to save for a child's educational expenses. Please read the Education Savings Account Disclosure Statement (attached to the application) for additional information and before opening an Education Savings Account. 403(b) Account
Due to changes in government regulations, the Sound Mind Investing Funds are no longer able to open new 403(b) accounts. All accounts opened prior to the changes will continue to be serviced as before. If you have a 403(b) account from a former employer, you may "roll it over" into a new IRA account with the Sound Mind Investing Funds. If you wish to do that, please see the prior section entitled "401(k), 403(b) or 457(b) Rollover" for instructions.

Automatic Investment / Withdrawal Plans
These forms are used to establish an automatic investment or withdrawal plan, allowing you to move money between your bank account and mutual fund account on a regular and automated basis. Note that these forms require a signature guarantee, which is easily obtainable at most commercial (FDIC) banks.
